JUNIOR CLUB MEMBERSHIP APPLICATION INSTRUCTIONS

This form is for teachers registering their studios as Junior Clubs. Junior clubs are for students ages 18 and under. If you have an adult student that would like to participate MFMC or NFMC events (Festival, etc.), they need to become a senior member of a club.
Fill out the application form.
Using this link, open the template. Click on “File” and “Make a Copy, then rename it with the club’s full name. Fill out the required information.
Email a copy of the completed spreadsheet to michiganfederationofmusicclubs@gmail.com before the due date, or include the appropriate late fee with your payment. Spreadsheets must be submitted by email in .xlsx or .ods format.
